It is important that these forms are completed before school starts, as it contains medical information, who is authorized to pick up your student/s, etc.  Some are required and some are for reference only.  Paper copies are available in the SMNS office and will be available on Welcome Back Night in the main Octagon of Riveredge Nature Center.  Email electronic complete forms to: roles@nosd.edu
